利用LIS系统样本时间流转功能保证时限性样本检
  [摘要]目的 探讨利用LIS系统样本时间流转功能保证时限性样本检测质量的价值。方法 选取2017年7~12月和2017年1~6月吉林市人民医院检验科住院部收集的尿常规、血氨和血气分析作为时限性样本,分别有25 679、912、5117例和24 313、867、4895例。2017年1~6月采用常规标准操作流程检测时限性样本,2017年7~12月在常规标准操作流程基础上增加预警时间设置:尿常规的预警时间为90 min,血氨为30 min,血气分析为10 min。比较不同时间段内时限性样本周转时间符合率、中位数和90%位数。结果 2017年7~12月,尿常規、血氨和血气分析的TAT符合率分别为98.7%、97.1%和98.3%,明显高于2017年1~6月的90.5%、80.5%和88.6%(P<0.05)。2017年7~12月,尿常规的TAT中位数、90%位数分别为32.6、62.5 min,血氨为20.6、45.8 min,血气分析为20.6、27.8 min均短于2017年1~6月的48.5、88.6 min,32.6、55.7 min,23.8、29.1 min(P<0.05)。结论 LIS系统样本时间流转功能可以保证时限性样本的时间检测需求和检测质量。

  [Abstract] Objective To explore the value of using LIS system sample time flow function to guarantee the inspection quality of time limited samples. Methods The urine routine, blood ammonia and blood gas analysis collected by inpatient department of department of clinical laboratory in People′s Hospital of Jilin City from July to December 2017 and from January to June 2017 were selected as time-limited samples, and there were 25 679, 912, 5117 cases, and 24 313, 867, 4895 cases. From January to June 2017, the routine standard operation process was used to detect the time limited samples while from July to December 2017, the setting of pre-warning time was added on the basis of routine standard operation process: the pre-warning time of urine routine was 90 min, blood ammonia was 30 min, and blood gas analysis was 10 min. The coincidence rate, median and 90% digits of turnaround time (TAT) for time limited samples in different time periods were compared. Results The coincidence rates of TAT in urine routine, blood ammonia and blood gas analysis from July to December 2017 was 98.7%, 97.1% and 98.3% respectively, which was significantly higher than that from January to June 2017 (90.5%, 80.5% and 88.6%) respectively (P<0.05). From January to June 2017, the median and 90% digits of TAT in urine was 32.6, 62.5 min rspectivley, blood ammonia was 20.6, 45.8 min, and blood gas analysis was 20.6, 27.8 min, which were shorter than those from January to June 2017 (48.5, 88.6 min; 32.6, 55.7 min; 23.8, 29.1 min) (P<0.05). Conclusion The sample time flow function of the LIS system can guarantee the time detection requirement and detection quality of time limited samples.

  2015年原国家卫生和计划生育委员会印发关于临床检验专业医疗质量控制标准的15个指标中,多个质量指标均与标本周转时间(turnaround time,TAT)相关[1],TAT不仅是医院作为服务行业满意度的客观指标,更是时限性样本检测质量的重要保证[2]。时限性样本如果出现超时,将严重影响检验结果的准确性,从而误导临床医生对病情的诊断和治疗[3]。通常医院检验科面对多达上千样本的检测,如何保证时限性样本检测的时间需求是医疗工作者关注的重点,本研究旨在探讨利用实验室信息管理系统(LIS)样本时间流转功能保证时限性样本的检测质量,现报道如下。
